I have been nursing for almost 25 years. I started my nursing career in Fiji as a Registered Nurse in 2001 and worked there for 6 years. I migrated to New Zealand in 2007 and worked across different health sectors for 11 years, then moved to Australia in 2019. Since 2019, I have been working at IFamily Medical Centre as a Registered Nurse. Qualification: Bachelor of Nursing Job Description: Immunisation Cold chain / vaccine management Infection prevention, control and sterilisation Chronic disease management – diabetes, asthma, hypertension, heart disease, arthritis, epilepsy, COPD and autoimmune disorders Utilisation of registers, reminders and recalls for patient care Preventive health screening and assessments Clinical procedures – spirometry, ECG, wound management, triaging, suture removal and assisting in minor surgical procedures Stock management Front desk and admin support – fax, emails and phone consults NDIS & Aged Care – Key Responsibilities: Provide clinical nursing care to aged care and NDIS clients. Conduct health assessments and monitor patient conditions. Develop and implement individualised care plans. Administer medications and manage treatment plans. Support clients with chronic disease management. Assist with wound care and post-hospital recovery. Monitor and report changes in client health status. Educate clients, families and carers on health management. Maintain accurate clinical documentation and progress notes. Liaise with multidisciplinary teams to coordinate care. Aged Care: Support elderly clients with mobility, health monitoring and daily care. Assist with falls prevention and safety planning. Manage chronic conditions such as arthritis, diabetes and cardiovascular issues. Deliver compassionate, patient-centred care to improve quality of life. NDIS: Support NDIS participants in achieving their health and functional goals. Assist with care plans aligned to NDIS funding and goals. Collaborate with support coordinators, families and allied health professionals. Provide clinical input for reports and progress updates.
